Complete this application form for the 2012 MLK Girls/Women's Invitational Tournament1) Complete blue fields below
2) Within 72 hours of receipt of application, the primary contact will receive a token by email on your conditional acceptance and directions on how to pay the tournament fee online by credit card.
3) Once payment has been made, teams may complete the registration process, enter rosters and submit a 5"x7" Team Photo (.jpg) and team logo (.jpg)

Payment Instructions:
Tournament Fee is $1150 for Girls Divisions. $1000 for the Girls 8u & the Women's Divisions.
Payment is to made by credit card at the link provided to your primary contact. Acceptance to the tournament is not complete until payment has been made. Note: Make sure you make and save a copy of your confirmation of payment.
