Registration

General Registration Process

1.   Download, complete and sign the following forms.  Bring these completed forms to tryouts:

• Tryout Registration Form
• Consent to Treat
• Waiver of Liability
• Code of Conduct

2.    Registration for USA Hockey current and following year are required for tryouts.  (e.g. tryouts in 2011 require USA Hockey registration for 2011-2012)

3.    Approximate Tryout fees: $190 for first tryout, $70 for additional Jr. Shark tryouts.

4.    Tryouts schedule is posted approximately one month before tryouts.
